When an elderly person goes to the hospital, age can impact their recovery. Now, some hospitals are putting a team approach to work for these patients.
Some hospitals have created ACE units OR Acute Care for Elders.
The ACE team coordinates all aspects of a patient's care, starting with a daily team meeting.
"It prevents them from having to dig through a thick medical chart where they might get bits of information. Now they can actually talk to the therapist, talk to the nutritionist, talk to the social worker, talk to the nurse practitioner," said Dr. Kellie Flood.
ACE units have been around for about 20 years and are becoming more popular as our population ages.
This type of care doesn't cost more.
